Important Notes

Any application for licence renewal shall be made to the Director of Medical Services not later than 2 months before the expiry date of the current licence, failing which the applicant shall pay a late payment fee which shall be 20% of the fee payable for the licence renewal or $100, whichever is the greater. [Regulations 6(1) and 6(2) of the PHMC Regulations].

Any amendment to a licence which has been issued shall be made by way of an application for a fresh licence and shall be accompanied by the appropriate fee specified in the Fourth Schedule [paragraph 5(7) of the PHMC Regulations]. As such, applicants are to ensure that all the information provided in their applications are correct and updated before proceeding with the submission in eLA.

All existing licensees are required to notify MOH of any change in the contact information on the person responsible for receiving medalert within 24 hours of such change [Regulation 4(1)(b) of the PHMC (MedAlert System) Regulations 2008].
